2018 TFA Award Nomination Form
Awards will be given in three (3) categories at this year's TFA Conference.
1. Aging Innovation Award (1 Award per Grand Division given)
3. Legislative Aging Champion Award (1 Award per Grand Division given)
All nominees must have been active within the last year. Winners will be recognized at the TFA Annual Awards Luncheon on November 28, 2018 at the Music Road Hotel in Pigeon Forge, TN.
Deadline to submit completed applications is Friday, October 19, 2018.

1. Aging Innovation Award (1 Award per Grand Division given)
- Project/Program must have met 1 or more of the following criteria:
- Recognized in the community (by mayor, commissioners, clubs, etc.)
- Established new partnerships
- Offered new/different service or program within the last year
- Improved the lives of older adults in the community
- Must have provided volunteer services to, or on behalf of, Older Adults
3. Legislative Aging Champion Award (1 Award per Grand Division given)
- Current State Legislator who advocates for Older Adult issues
All nominees must have been active within the last year. Winners will be recognized at the TFA Annual Awards Luncheon on November 28, 2018 at the Music Road Hotel in Pigeon Forge, TN.
